Roles & Responsibilities

Provide compassionate direct patient care and support daily living needs
Administer medications and treatments accurately per care plan
Monitor and document patient status, reporting changes promptly
Collaborate with RNs and interdisciplinary team to deliver safe care
Assist with admissions, discharges and patient transfers smoothly
Maintain a clean, safe environment and follow infection control practices
Engage patients and families with clear, respectful education and communication

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania
Active BLS/CPR certification (or willingness to obtain)
Comfortable with medication administration, charting, and basic clinical skills
Strong communication and teamwork mindset
Previous experience as an LPN preferred; new grads with clinical training are welcome to inquire

Education

Diploma or certificate from an accredited practical nursing program
State LPN licensure required

Knowledge & Skills

Familiarity with basic nursing assessments and vital signs
Ability to accurately document care in electronic or paper charts
Medication administration knowledge and safe practice
Patient-centered care and strong interpersonal skills
Time management and ability to prioritize in a busy setting
