Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients, ensuring comfort and dignity
• Administer medications and treatments accurately and document them thoroughly
• Monitor and record vital signs, observing and reporting changes promptly
• Assist patients with activities of daily living and mobility as needed
• Collaborate with RNs and interdisciplinary team members to create and follow patient care plans
• Maintain clear, timely charting in the patient record and communicate effectively with families
• Follow infection control protocols and safety procedures to protect patients and staff
• Contribute to a positive care environment by supporting team initiatives and mentoring peers

Qualifications

• Active LPN (Licensed Practical Nurse) license in Pennsylvania (required)
• Graduate of an accredited practical nursing program
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Comfortable with medication administration, patient assessments, and documentation
• Strong communication and teamwork skills; patient-focused and reliable
• Ability to pass background check, drug screening, and meet facility onboarding requirements

Education

• Graduate of an accredited practical nursing (LPN) program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and accurate charting
• Patient assessment and basic clinical decision-making
• Familiarity with electronic health records (EHR) documentation
• Infection control standards and safe patient handling
• Effective communication with patients, families, and clinical teams
• Time management and prioritization in a fast-paced care setting
