Roles & Responsibilities

Provide compassionate direct patient care including medication administration and basic treatments
Monitor and record vital signs and patient status, escalating concerns to the care team
Maintain accurate and timely documentation of patient care activities
Collaborate with nurses, therapists, and providers to deliver coordinated care
Assist with patient hygiene, mobility, and comfort to promote dignity and recovery
Follow infection control and safety protocols to protect patients and staff
Educate patients and families on care plans and follow-up needs in a clear, caring way

Qualifications

Licensed Practical Nurse (LPN) credential in good standing
Current, active LPN license (ability to obtain/maintain state licensure as required)
Basic patient care skills and the ability to work 8-hour shift assignments
Strong communication and teamwork skills to work effectively with the care team
CPR/BLS certification preferred or willingness to obtain

Education

Completion of an accredited LPN/Licensed Practical Nurse program

Knowledge & Skills

Medication administration and basic clinical assessment
Vital signs monitoring and documentation best practices
Patient hygiene, mobility assistance, and wound care basics
Electronic charting and accurate record keeping
Infection prevention and safety procedures
Professional communication and patient education skills
