Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and document thoroughly
• Monitor patient status, record vital signs, and report changes to the care team
• Collaborate with nurses, physicians, and support staff to deliver coordinated care
• Educate residents and families about care plans and encourage participation
• Assist with admissions, transfers and safe patient discharges when needed
• Maintain accurate medical records and follow infection control and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing (required)
• Active nursing license applicable to practice in New Hampshire (or ability to obtain)
• Previous LPN experience in long-term care or similar settings preferred
• Reliable, professional demeanor with strong communication and teamwork skills
• Willingness to complete facility orientation and required credentialing

Education

• Diploma or certificate from an accredited Practical Nursing program

Knowledge & Skills

• Strong medication administration and documentation skills
• Familiarity with basic wound care and vital sign assessment
• Comfortable working with electronic medical records and charting
• Ability to prioritize tasks and manage time effectively on a busy unit
• Excellent interpersonal skills and resident-centered care approach
• Knowledge of infection control practices and safety standards
