Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and on schedule
• Monitor and document patient vital signs, responses, and progress
• Collaborate with the interdisciplinary care team to deliver coordinated care
• Maintain clear, timely electronic and paper documentation
• Support patient comfort, safety, and dignity with attentive bedside care
• Follow facility policies and evidence-based practices to ensure quality care
• Communicate with families and care team members to keep everyone informed

Qualifications

• Current, active LPN license valid in Pennsylvania (required)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working 12.5-hour shifts and managing patient care across a full shift
• Strong ability to prioritize, multitask, and remain calm under pressure
• Excellent communication skills and a team-focused attitude
• Previous LPN experience in a long-term care or skilled nursing setting preferred

Education

• High school diploma or GED
• Completed LPN program (diploma or certificate) and active LPN licensure

Knowledge & Skills

• Medication administration and documentation best practices
• Skilled in basic nursing assessments and monitoring vital signs
• Familiarity with electronic health records (EHR) and clinical charting
• Infection control, wound care basics, and safe patient handling
• Ability to follow care plans and communicate changes effectively
• Customer-service mindset when interacting with residents and families
