Roles & Responsibilities

• Provide compassionate direct patient care, assisting with activities of daily living
• Perform safe patient transfers and mobility assistance with proper body mechanics
• Take and record vital signs and report changes in patient condition promptly
• Document care accurately and clearly in the facility charting system
• Support infection control practices and maintain a clean, safe environment
• Work collaboratively with nurses and the care team to implement care plans

Qualifications

• Current LNA license in New Hampshire or eligible to obtain state LNA licensure
• Completion of a state-approved nursing assistant training program
• Basic Life Support (BLS/CPR) certification preferred or ability to obtain upon hire
• Comfortable assisting with personal care tasks and mobilizing patients
• Strong communication skills and a positive, team-oriented attitude
• Previous experience in long-term care or acute care preferred but new grads welcome

Education

• High school diploma or GED
• Graduation from a state-approved nursing assistant program
• Current LNA certification/licensure

Knowledge & Skills

• Fundamentals of patient safety, infection control and standard precautions
• Ability to measure and record vital signs accurately
• Skilled in assisting with bathing, grooming, feeding and toileting
• Basic charting and documentation skills
• Comfortable using lifting and transfer equipment
• Teamwork, time management and clear communication with patients and families
