Roles & Responsibilities

Conduct thorough psychosocial assessments to understand patients' mental, emotional, and social needs
Develop individualized treatment plans that are evidence-based and culturally sensitive
Deliver therapeutic interventions across the lifespan for issues such as depression, anxiety, trauma, substance use, and adjustment disorders
Collaborate closely with primary care providers, nurses, and behavioral health professionals to coordinate whole-person care
Maintain clear, timely documentation to support continuity of care and compliance with clinical standards
Advocate for patients in a rural, medically underserved setting and connect them with community resources

Qualifications

Active LISW license in Iowa (required)
Master's degree in Social Work from an accredited program (required)
Minimum of 2 years of clinical experience preferred
Experience working in rural or medically underserved settings is a plus
Strong interpersonal and organizational skills with excellent documentation practices
Commitment to working as part of an integrated care team and serving diverse patient populations

Education

Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

Psychosocial assessment and formulation
Treatment planning and progress monitoring
Evidence-based therapeutic interventions (individual and group modalities)
Integrated care practices and interdisciplinary collaboration
Behavioral health diagnostics and interventions across the lifespan
Clinical documentation and electronic health record familiarity
Strong communication, organization, and case management abilities
