Roles & Responsibilities

Provide compassionate direct patient care, assisting with daily living activities
Monitor and record vital signs and report changes to the nursing team
Assist with mobility, transfers, and safe patient handling
Maintain accurate patient documentation and care logs
Support infection control practices and maintain a clean patient environment
Collaborate with nurses and interdisciplinary staff to deliver quality care
Build trusting relationships with residents and their families

Qualifications

Current CNA certification (required)
Active healthcare-related licenses or certifications as applicable in Pennsylvania
Ability to pass background check and any required health screenings
Comfortable working in a long-term care or similar healthcare setting
Strong communication skills and a compassionate bedside manner

Education

High school diploma or GED preferred
CNA certification required

Knowledge & Skills

Patient care fundamentals and activities of daily living (ADLs)
Vital signs measurement and basic clinical observation
Infection prevention and proper hand hygiene practices
Safe transfer and mobility assistance techniques
Clear, timely documentation and handoff communication
Teamwork and the ability to follow care plans and nurse instructions
