Roles & Responsibilities

• Provide compassionate personal care and assist residents with activities of daily living
• Monitor and report changes in resident condition to nursing staff promptly
• Support safe transfers, mobility and fall prevention efforts
• Maintain accurate documentation and follow care plans consistently
• Assist with mealtime, grooming, hygiene and toileting while preserving dignity
• Collaborate with the care team to create a warm, home-like environment

Qualifications

• Current Certified Nursing Assistant (CNA) certification
• State authorization or eligibility to practice as a CNA in Pennsylvania
• Comfortable working evenings and weekends on the posted schedule
• Strong communication skills and a patient-centered approach
• Basic infection control knowledge and commitment to safety

Education

• High school diploma or GED preferred

• Hands-on resident care and assistance with ADLs
• Safe transfer and mobility support
• Accurate charting and communication with interdisciplinary teams
• Infection prevention and basic wound care awareness
• Time management, teamwork and compassionate bedside manner
