Roles & Responsibilities

• Provide compassionate, hands-on nursing care to assigned patients
• Administer medications and treatments per physician orders and facility policy
• Perform thorough patient assessments and document changes promptly
• Coordinate with the care team to support safe, effective discharge planning
• Monitor patient condition and escalate concerns to RN or provider as needed
• Support patient comfort, dignity, and education during each shift

Qualifications

• Active Licensed Practical Nurse (LPN) credential in good standing
• Current BLS/CPR certification (or ability to obtain upon hire)
• Experience in long-term care, med-surg, or similar settings preferred
• Strong clinical judgment, clear documentation skills, and teamwork mindset
• Ability to work 12+ hour daytime shifts and adapt to per diem scheduling

Education

• Vocational/technical diploma or certificate in Practical Nursing (LPN)

Knowledge & Skills

• Medication administration and basic IV/IM skills where applicable
• Vital signs monitoring, wound care basics, and ADL assistance
• Clear and compassionate communication with patients and families
• Familiarity with electronic health records and charting best practices
• Time management and the ability to prioritize in a busy environment
