Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to PACE participants
- Administer medications and treatments safely and accurately
- Perform routine assessments and document clinical findings in the chart
- Collaborate with the interdisciplinary team to support participant health goals
- Assist with activities of daily living and encourage independence
- Follow infection control policies and ensure participant safety
- Support smooth transitions of care and communicate changes to the care team
Qualifications
- Current Licensed Practical Nurse (LPN) license, or eligible for licensure in Louisiana
- Current CPR certification (or obtain upon hire if required)
- Up-to-date immunizations as listed by employer: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
- Clear Trinity 10 panel drug screen and completed physical/health statement
- Comfortable working in a per diem PD Pool model and adapting to varied assignments
- Strong communication skills and a heart for working with older adults
Education
- Graduate of an accredited practical nursing program, or equivalent
- Active LPN licensure required
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic assessment skills and recognizing changes in condition
- Familiarity with infection prevention and standard precautions
- Comfortable with electronic health records and accurate charting
- Team collaboration and clear, professional communication
- Ability to prioritize tasks and manage time during variable shifts
