Roles & Responsibilities
- Provide compassionate direct patient care, including medication administration and skilled nursing tasks
- Monitor patient status, document changes, and escalate concerns to the RN or provider
- Assist with ADLs and encourage patient independence while maintaining dignity
- Collaborate with the clinical team to implement and update individualized care plans
- Perform routine assessments, wound care, and treatments as ordered
- Maintain accurate, timely charting in the facility’s medical record system
Qualifications
- Current LPN license in good standing (Maryland license preferred)
- Current CPR/BLS certification
- Completion of an accredited Practical Nursing program
- Prior experience in long-term care or skilled nursing is preferred
- Strong communication skills and a patient-centered mindset
Education
- Graduate of an accredited Practical Nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and documentation
- Basic wound care and dressing changes
- Vital signs monitoring and clinical assessment
- Familiarity with electronic medical records and charting
- Infection control and standard precautions
- Time management and teamwork in a clinical setting
