Location: Prescott, Arizona

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, weekends every 3-4 weeks if at all

Hours per Week: 40

Candidate Pay Rate: $50 per hour

Roles & Responsibilities

Complete comprehensive social work assessments to understand each patient's unique needs
Conduct mental health assessments and provide crucial support during vulnerable moments
Coordinate discharge planning to ensure smooth transitions of care
File Title 36 petitions and support patients experiencing homelessness or substance abuse challenges
Address social determinants of health through thoughtful consultations
Perform maternal child assessments and connect families with vital resources
Report to DCS and APS as needed to protect vulnerable populations

Qualifications

Active LMSW or LCSW license in Arizona
Experience with Cerner EMR preferred
Strong assessment and discharge planning skills
Compassionate approach to diverse patient populations

Knowledge & Skills

Social Work Assessment expertise
Mental Health Assessment proficiency
Discharge Planning experience
Knowledge of Title 36 processes
Understanding of homelessness and substance abuse resources
Familiarity with SDOH consultations
Maternal child assessment skills
DCS and APS reporting knowledge
