Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care assessments and develop personalized treatment plans
Perform recertifications and resumption of care visits
Complete 6.5 points per day with a mix of regular visits and admissions
Collaborate with clinical coordinators and sister agency therapists for support
Document patient progress using OASIS standards
Ensure safe and effective care delivery in home settings

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid Driver's License and Auto Insurance
Home Health experience preferred
OASIS training preferred
First time travelers welcome to apply

Knowledge & Skills

Physical Therapy expertise in home health settings
OASIS documentation proficiency
Strong communication and patient education skills
Ability to work independently in the field
Time management for 5-6 visits per day
