Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct start of care assessments and evaluations for new patients
Perform recertifications and resume of care visits as needed
Deliver personalized physical therapy treatment in patients' homes
Manage a caseload of patients with CHF, orthopedic conditions, and pneumonia
Collaborate with clinical managers and educators for optimal patient outcomes

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but not required
Valid driver's license and reliable transportation

Knowledge & Skills

Strong clinical assessment and treatment planning abilities
Excellent communication skills for patient and family education
Comfortable working independently in home settings
Proficiency with electronic documentation systems
Compassionate approach to geriatric and medically complex patients
