Roles & Responsibilities

Deliver personalized physical therapy to patients recovering from falls and fractures
Complete OASIS assessments and Start of Care evaluations with accuracy and compassion
Manage a manageable caseload of 4-6 visits daily across four rural counties
Collaborate with an interdisciplinary team to create tailored care plans
Document visits efficiently using provided tablet technology

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable traveling between rural communities and small towns
Strong communication and time-management skills
