Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide personalized physical therapy services to patients recovering from surgeries, falls, and other conditions in their homes
Develop individualized treatment plans that help patients regain mobility and independence
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Document patient progress using HomeCare/HomeBase EMR system
Educate patients and families on exercises and safety techniques to support recovery

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
Minimum 1 year of physical therapy experience with previous home health experience preferred
Valid driver's license and auto insurance
Strong communication and interpersonal skills

Knowledge & Skills

Home health physical therapy expertise
Experience with post-surgical and fall recovery patients
Proficiency with HomeCare/HomeBase EMR
Ability to work independently in home settings
Compassionate approach to patient-centered care
