Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate care coordination for patients in need of mental health support
Conduct thorough assessments and develop individualized care plans
Collaborate with interdisciplinary teams to ensure seamless patient care
Support patients and families through challenging healthcare journeys
Maintain accurate documentation while delivering exceptional service

Qualifications

Active LCSW license in California
Current BLS certification
5150 certification in California
Prior experience with Dignity Health facilities
Strong clinical assessment and care coordination skills

Knowledge & Skills

Expertise in involuntary psychiatric evaluation processes
Understanding of 72-hour mental health holds
Ability to work collaboratively within care teams
Strong communication and documentation abilities
