Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, 8-hour day shifts

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Manage a meaningful caseload of 5-6 new patients and 3-4 follow-up visits daily
Lead compassionate goals of care discussions with patients and families
Educate patients on disease processes, advance directives, and POLST documentation
Facilitate supportive family meetings during critical healthcare decisions
Provide end-of-life discussions and emotional support with dignity and respect
Collaborate with physicians and the interdisciplinary team during daily rounds
Review patient charts and assess individual needs with clinical expertise
Develop daily patient lists to ensure comprehensive, coordinated care

Qualifications

Active California RN license required
2-5 years of recent palliative care or hospice experience
BLS certification required
ELNEC certification preferred
Acute care experience is essential for success in this role
ICU or oncology experience is a plus
Strong clinical thinking and communication skills
Flexibility and exceptional organizational abilities

Knowledge & Skills

Acute care clinical expertise
Compassionate communication with patients and families
Strong organizational and time management abilities
Cerner EMR proficiency
Understanding of palliative care principles and end-of-life care
Ability to navigate complex family dynamics with sensitivity
