Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, with rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists, ensuring comprehensive care coordination
Review patient charts and assess needs to tailor individualized palliative care plans
Facilitate goals-of-care discussions, empowering patients and families with knowledge
Educate on disease progression, advance directives, and POLST forms
Lead family meetings to provide support and navigate end-of-life decisions
Participate in daily interdisciplinary rounds, advocating for patient-centered care

Qualifications

2-5 years of recent experience in palliative care or hospice setting required
Acute care experience is a must
BLS certification required
ELNEC certification preferred (end-of-life care)
Strong clinical thinking, communication, and organizational skills
Flexibility and a genuine desire to serve others

Knowledge & Skills

Expertise in palliative and hospice care principles
Proficiency with Cerner EMR system
Strong acute care assessment and clinical thinking
Excellent communication for sensitive family discussions
Understanding of advance directives and POLST processes
Ability to work collaboratively in a multidisciplinary team
