Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; On-call rotation Friday-Sunday, 8:00 PM - 8:00 AM (day shift with rotating night on-call)
Hours per Week: 40
Candidate Pay Rate: $50 per hour

Roles & Responsibilities
- Provide compassionate hospice and home health care to primarily geriatric patients with a six-month or less prognosis
- Perform intake assessments, symptom assessments, and ongoing skilled nursing visits that prioritize comfort and dignity
- Manage pain using WHO analgesic ladder principles, breakthrough dosing, and both pharmacologic and non-pharmacologic interventions
- Educate and support patients and families on care goals, medication management, positioning, and pressure ulcer prevention
- Administer medications safely, including pediatric dose calculations when needed, and ensure proper disposal
- Identify and manage common end-of-life symptoms such as altered mental status, nausea/vomiting, constipation, anorexia/cachexia, and ascites
- Coordinate with interdisciplinary team members and respond to triage calls to determine when in-home visits are necessary
- Document thoroughly in Epic and follow Medicare/state regulations and agency policies
Qualifications
- Active RN license in state or ability to obtain licensure in North Carolina
- Minimum 2 years of relevant experience in Home Health, Hospice, or Skilled Nursing (unit accepts first-time travelers)
- BLS certification required
- Comfort with on-call responsibilities (Friday–Sunday night rotations) and ability to respond to PRN/death visits
- Experience or comfort with pain management protocols, pediatric dose calculations, and pressure ulcer staging
- Epic charting experience preferred (home health/hospice Epic a plus)
- Professional communication skills for goal-setting with patients/families and interdisciplinary collaboration
Knowledge & Skills
- Home Health/Hospice nursing principles and end-of-life care
- Medication administration and pain management (WHO analgesic ladder, adjuvant therapy, breakthrough dosing)
- Pain assessment and pharmacologic/non-pharmacologic interventions
- Symptom assessment: altered mental status, anorexia/cachexia, aphasia, constipation/diarrhea, nausea/vomiting, ascites
- Pressure ulcer staging and prevention, positioning and preventive shearing
- Intake assessment/interviewing and Medicare/state regulations
- PCA pump management, equianalgesic dosing, and safe medication disposal
- Phlebotomy and IV management (peripheral IVs/central lines/ports) and basic DME authorization knowledge
- Familiarity with Epic charting and SBAR handoff communication
