Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with every other weekend rotation

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate comprehensive care for patients across all hospital units
Complete initial assessments and ongoing utilization reviews using Interqual criteria
Develop and implement effective discharge plans to ensure smooth transitions
Collaborate with interdisciplinary teams to prioritize patient needs daily
Manage a caseload of 15-20 patients per day with efficiency and compassion
Communicate with all payers, including uninsured and homeless populations

Qualifications

Current California RN license in good standing
BLS certification required
Prior recent Dignity Health experience is a must
Experience in case management, utilization review, and discharge planning
Ability to work every other weekend (at least one full weekend per month)

Knowledge & Skills

Proficiency with Interqual criteria and Cerner EMR
Strong collaboration and teamwork skills
Effective communication and ability to reprioritize throughout the day
Knowledge of all payer types, including government and uninsured
Understanding of hospital care coordination from admission to discharge
