Roles & Responsibilities

• Perform thorough patient evaluations to identify rehabilitation needs and goals
• Deliver daily therapeutic treatments focused on returning patients to home
• Plan, coordinate, and execute safe discharges with clear home transition plans
• Document progress and maintain clinical records in line with facility standards
• Collaborate with the interdisciplinary rehab team to optimize patient outcomes
• Maintain productivity expectations (90%) while providing high-quality care
• Adhere to Continuum Rehabilitation Group policies and facility procedures
• Represent the therapy team professionally (black scrubs dress code) and be present on site

Qualifications

• PT or DPT credential (PT/DPT listed as required skills)
• 1-3 years of clinical experience (as noted in the posting)
• Prior SNF experience strongly preferred
• Ability to work onsite and meet productivity and policy standards
• Willingness to follow facility dress code and CRG policies

Education

• DPT (Doctor of Physical Therapy) or equivalent PT credential

Knowledge & Skills

• Skilled in patient evaluations, treatment planning, and daily therapeutic interventions
• Experience managing discharges and preparing patients for rehab-to-home transitions
• Familiarity with skilled nursing facility workflows and documentation practices
• Strong teamwork, communication, and time-management skills
• Comfort working to productivity targets while maintaining patient-centered care
