Roles & Responsibilities

• Provide hands-on physical therapy interventions under PT supervision to help patients regain mobility and independence
• Deliver individualized treatment plans focused on functional outcomes for SNF residents
• Assist with therapeutic exercise, gait training, transfers and use of assistive devices
• Monitor patient progress, document outcomes accurately, and communicate changes to the supervising PT
• Collaborate with nursing and interdisciplinary staff to support safe discharge planning and continuity of care
• Educate patients and families on home programs and strategies to maintain gains

Qualifications

• 1-3 years of PTA experience (as listed in the job posting)
• Experience working in a Skilled Nursing Facility (SNF) setting preferred
• State PTA licensure or eligibility to obtain licensure prior to start (typical requirement for practice)
• Current BLS/CPR certification (or willingness to obtain) is preferred
• Strong communication, documentation, and patient-centered care skills

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise and mobility training
• Knowledge of rehab principles, falls prevention and adaptive equipment
• Proficient documentation and clinical reporting for SNF settings
• Ability to work collaboratively within an interdisciplinary team
• Comfortable managing varied caseloads and prioritizing patient needs
