Roles & Responsibilities

• Provide skilled nursing care across long-term care and acute/swing bed settings
• Lead and supervise CNA staff to ensure safe, compassionate patient care
• Perform rounds and nighttime toileting assistance with dignity and respect
• Collaborate with the RN and interdisciplinary team to manage patient needs
• Document patient assessments, interventions, and observations accurately in the chart
• Administer medications and treatments per scope of practice and facility policy
• Support orientation and training for new staff to keep standards high

Qualifications

• Current LPN licensure in the state of practice (or compact/reciprocal as applicable)
• Basic Life Support (BLS) certification — required
• Experience in long-term care (LTC) and acute/swing bed care
• Proven ability to supervise and mentor CNA staff
• Comfort with night-shift responsibilities and independent decision-making
• Team-player attitude and willingness to pitch in across departments

Education

• Licensed Practical Nurse (LPN) — vocational nursing diploma or equivalent

Knowledge & Skills

• Strong clinical assessment and patient monitoring skills
• Medication administration and basic wound care knowledge
• Familiarity with LTC routines, toileting assistance, and nightly rounds
• Effective communication with patients, families, and clinical staff
• Basic EMR/charting proficiency and attention to documentation detail
• Adaptability to rural healthcare settings and variable patient needs
