Roles & Responsibilities

• Provide high-quality physical therapy services to residents in a skilled nursing setting
• Assess patient function and create individualized treatment plans to restore mobility and independence
• Deliver skilled interventions and document progress in compliance with facility policies
• Collaborate with nurses, occupational therapists, speech therapists, and other team members to coordinate care
• Meet productivity expectations and contribute to a positive, efficient therapy workflow
• Maintain professional appearance and follow facility dress code (black scrubs)

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT)
• 1-3 years of experience preferred (SNF experience a plus)
• Comfortable working in a skilled nursing facility and collaborating with interdisciplinary teams
• Willingness to follow Continuum Rehabilitation Group policies and productivity standards
• Ability to be physically present at the facility to receive guaranteed hours

Education

• Doctor of Physical Therapy (DPT) or equivalent licensure as a Physical Therapist

Knowledge & Skills

• Skilled nursing facility care and short-term/long-term rehab principles
• Strong clinical assessment, treatment planning, and documentation skills
• Good communication and teamwork abilities for interdisciplinary collaboration
• Time management and productivity-focused approach to caseloads
• Familiarity with common geriatric diagnoses, mobility assistance, and therapeutic modalities
