Roles & Responsibilities

• Perform thorough physical therapy evaluations and document findings clearly
• Develop individualized plans of care and adjust them based on patient progress
• Provide treatment across settings: patient rooms, hallways, outdoor areas, and the rehab gym
• Collaborate with the multidisciplinary rehab team and participate in rounds and care conferences
• Communicate frequently about equipment needs, discharge planning, and progress
• Float between acute/swing beds, nursing home, and outpatient clinics as needed
• Document care accurately using Cerner and NetHealth systems

Qualifications

• Current NC Physical Therapy license (required)
• Current CPR certification (required)
• Experience in a hospital and/or nursing home setting (must have)
• Prefer at least 3 years of experience in a medical setting
• Comfortable working with swing bed patients and in a rehab gym environment
• Familiarity with Cerner and NetHealth documentation systems preferred

Knowledge & Skills

• Skilled in physical therapy evaluations and developing plans of care
• Strong rehab-focused clinical skills across acute, swing, skilled nursing, and outpatient settings
• Ability to safely manage 1:1 patient care and adapt treatment environments
• Experience with typical rehab equipment and therapeutic modalities
• Proficient documentation practices using Cerner and NetHealth
• Excellent communication and teamwork skills within a multidisciplinary rehab department
