Roles & Responsibilities

• Assess patients’ physical function and mobility to craft individualized treatment plans
• Develop and implement evidence-based interventions to improve strength, balance, and independence
• Document patient progress clearly and timely in the electronic medical record
• Collaborate with interdisciplinary team members to coordinate high-quality care
• Educate patients and families on home programs and fall-prevention strategies to support lasting gains
• Monitor outcomes and adjust plans of care to ensure measurable patient progress
• Assist with discharge planning to ensure safe transitions back to the community

Qualifications

• Valid Physical Therapist license in New Hampshire, or eligibility to obtain licensure quickly
• Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Experience in inpatient or outpatient rehab preferred; new grads may be considered depending on facility needs
• Strong communication skills and a collaborative attitude
• Ability to document accurately and use electronic medical records

Education

• Doctor of Physical Therapy (DPT) preferred
• Bachelor’s or Master’s in Physical Therapy or related field accepted

Knowledge & Skills

• Skilled in assessment techniques, therapeutic exercise, and mobility training
• Experience with manual therapy and assistive device training
• Familiarity with common modalities and rehab equipment
• Good clinical reasoning and outcome-driven treatment planning
• Comfortable working in an interdisciplinary team environment
• Strong documentation skills and basic computer/EMR navigation
