Roles & Responsibilities

• Assess wounds, ostomies, fistulas and pressure ulcers using best-practice methods
• Develop and implement individualized wound care plans that improve healing and patient comfort
• Educate patients and families on dressing techniques, wound care routines and equipment use
• Recommend appropriate wound and ostomy supplies and equipment to optimize outcomes
• Document assessments, interventions and progress clearly and timely in the patient record
• Collaborate with interdisciplinary teams to coordinate comprehensive patient care
• Monitor and escalate changes in wound status or patient condition to appropriate providers

Qualifications

• Wound Care Certification — mandatory (as stated in posting)
• Minimum 3 years of wound care experience (mandatory)
• American Heart Association BLS — required
• ACLS — preferred
• Proven experience with ostomy care, fistula management and pressure ulcer treatment
• Strong patient education and care-planning skills

Knowledge & Skills

• Wound assessment and staging
• Pressure ulcer prevention and management
• Ostomy and fistula care techniques
• Care planning and implementation for acute and chronic wounds
• Patient and family education for home wound/ostomy care
• Selection and recommendation of wound care supplies and equipment
• Accurate clinical documentation and care coordination
• Basic life support (BLS) competency; ACLS knowledge a plus
