Roles & Responsibilities

• Provide compassionate direct patient care in a long-term care setting, focusing on resident comfort and safety
• Perform nursing assessments and document findings in PCC to maintain accurate resident records
• Administer medications and treatments according to facility protocols and care plans
• Collaborate with the interdisciplinary team and float between two floors as staffing needs require
• Support a positive resident experience through effective communication with residents and families
• Follow infection control and safety standards to protect residents and staff

Qualifications

• Active Licensed Practical Nurse (LPN) credential (LPN)
• Experience or interest in Long-Term Care (LTC) environments, including PCC charting
• Comfortable working 12-hour day shifts (8:00 AM - 8:00 PM)
• Ability to float between units/floors and adapt to changing assignments
• Strong communication and teamwork skills to work with nursing staff and leadership

Education

• Completion of an accredited practical nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Proficient in patient care fundamentals and LTC nursing practices
• Familiarity with PCC electronic charting is a plus
• Time management and prioritization for 12-hour shifts
• Ability to work effectively while floating between units
• Resident-centered care mindset and strong observational skills
