Physical Therapist - Skilled Nursing Facility
Location: Lancaster, CA
Contract Type: Contract
Candidate Pay Rate: $62 - $65 per hour

Roles & Responsibilities
- Evaluate and assess patients' physical conditions to create clear baselines and measurable goals
- Develop and implement individualized treatment plans that help patients regain mobility and independence
- Provide therapeutic exercises and mobility training with a hands-on, patient-centered approach
- Monitor and document patient progress clearly and thoroughly to support continuity of care
- Collaborate with physicians, nurses, and interdisciplinary teams to deliver coordinated treatment
- Ensure compliance with facility policies and regulatory standards to maintain high-quality care
Qualifications
- Active California Physical Therapist License (required)
- Doctor of Physical Therapy (DPT) or equivalent
- Current BLS certification
- SNF experience preferred but enthusiastic, patient-focused clinicians are encouraged to apply
- Strong clinical reasoning and documentation skills
Education
- Doctor of Physical Therapy (DPT) or equivalent
Knowledge & Skills
- Skilled in patient assessment and developing individualized treatment plans
- Experienced with therapeutic exercises and mobility training
- Comfortable documenting progress and communicating with interdisciplinary teams
- Knowledgeable about SNF workflows, regulatory standards, and quality documentation
- Strong interpersonal skills and patient-focused care delivery
