Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop personalized care plans
Guide patients and families through exercises and the safe use of therapeutic equipment
Monitor progress and communicate updates with physicians to ensure optimal outcomes
Collaborate with home health aides and caregivers to support rehabilitation goals
Document clinical findings and progress summaries to support quality care

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Therapeutic exercise instruction
Prosthetic device training
Compliance with Medicare and state regulations
