Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients in their homes across a 60-mile radius
Help patients recover from surgeries, falls, and other conditions with tailored treatment plans
Collaborate with home health and hospice interdisciplinary teams to coordinate comprehensive care
Document patient progress using HomeCare/HomeBase EMR system
Educate patients and families on exercises and safety techniques to support recovery
Manage a caseload of 5-7 patients per day while maintaining high-quality care standards

Qualifications

Current North Dakota Physical Therapy License required
BLS certification required
1+ year of previous home health experience preferred
Valid driver's license and auto insurance required
Strong PT clinical skills with a passion for home-based care

Knowledge & Skills

Physical Therapy expertise in home health settings
HomeCare/HomeBase EMR proficiency
Excellent communication and patient education skills
Ability to work independently while traveling within the coverage area
Compassionate approach to caring for home health and hospice patients
