Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care assessments, recertifications, and resumption of care visits
Deliver 5-6 meaningful patient visits per day, making a real difference in each home
Collaborate with a supportive clinical team including coordinators and directors
Complete safety evaluations and occasional hospice teaching visits when needed
Document care using OASIS and maintain accurate patient records

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid Driver's License and Auto Insurance required
Home Health experience and OASIS training preferred
First-time travelers welcome to apply

Knowledge & Skills

Physical Therapy expertise in home health settings
Comfortable completing start of care, recertifications, and daily visits
Familiarity with OASIS documentation
Strong communication and time-management skills
Ability to work independently while staying connected with your clinical team
