Roles & Responsibilities

Deliver personalized physical therapy care to patients across four rural counties
Conduct OASIS assessments and Start of Care evaluations that set the foundation for recovery
Help elderly patients regain strength and mobility after falls and fractures
Build trust and rapport with patients in their homes and community settings
Collaborate with an interdisciplinary team to create meaningful care plans

Qualifications

Active Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable providing care in rural and home health settings
Strong communication and time-management skills

Knowledge & Skills

OASIS assessment expertise
Start of Care evaluation skills
Home health and rural care experience
Tablet-based documentation proficiency
Patient-centered communication
