Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits, especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately five meaningful visits each day while building genuine connections with patients
Document care accurately using HomeCare HomeBase on the tablet provided by the facility

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than two years of experience if they have Home Health background
Comfortable traveling within the service area and managing a daily mileage of around 40 miles

Knowledge & Skills

Skilled in Home Health Physical Therapy practices
Proficient with HomeCare HomeBase (HCHB) EMR system
Strong assessment and care-planning abilities for orthopedic and geriatric patients
Excellent time management and independent decision-making skills
