Location: Louisville, Kentucky

Start Date: 06-22-2026

End Date: 09-21-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services in patients' homes
Conduct start of care assessments, recertifications, and evaluations
Develop personalized treatment plans for patients with CHF, orthopedic conditions, and pneumonia
Deliver daily therapy visits that help patients regain independence
Collaborate with clinical managers and educators for optimal patient outcomes
Document patient progress and adjust care plans as needed

Qualifications

Current Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but not required
Valid driver's license and reliable transportation
Strong communication and documentation skills

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
CHF, orthopedic, and pneumonia rehabilitation knowledge
Electronic documentation and tablet proficiency
Time management and independent decision-making
