Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct OASIS assessments and Start of Care evaluations for new patients
Help elderly patients recover from falls and fractures with personalized care plans
Manage a manageable caseload of 4-6 patient visits daily using a point system
Document visits and track time efficiently with provided tablet technology

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural community settings
Strong organizational skills for managing daily patient visits

Knowledge & Skills

OASIS assessment expertise
Start of Care evaluation skills
Rural healthcare experience preferred
Tablet-based documentation proficiency
Geriatric rehabilitation knowledge
