Contract Type: Travel Contract

Shift Type: 8:00 AM - 4:30 PM, Monday-Friday

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct comprehensive physical therapy evaluations for acute, swing bed, nursing home, and outpatient patients
Develop and implement individualized plans of care to restore function and mobility
Collaborate with interdisciplinary team during rounds and care conferences to optimize patient outcomes
Document patient progress in Cerner and NetHealth systems with accuracy and timeliness
Communicate effectively with patients, families, and healthcare team regarding discharge needs and equipment
Provide treatment in patient rooms, hallways, outdoor areas, and the rehab gym

Qualifications

At least 2 years of experience in a hospital or nursing home setting (3+ years preferred)
North Carolina Physical Therapy license in hand (required)
Current CPR certification (required)
Proficiency in Cerner and NetHealth documentation systems
Ability to float between acute/swing, nursing home, and outpatient services as needed

Knowledge & Skills

Strong patient evaluation and care planning skills
Familiarity with typical rehab equipment and treatment techniques
Excellent communication and teamwork abilities
Adaptability to work across multiple care settings within the facility
