Roles & Responsibilities

• Provide one-on-one physical therapy to patients in their homes, helping them recover from surgeries, falls, and other conditions
• Develop and implement individualized treatment plans tailored to each patient's goals and abilities
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
• Educate patients and their families on exercises and strategies to promote independence and safety
• Accurately document patient progress and outcomes using HomeCare/HomeBase EMR system
• Travel within a 60-mile radius of Valley City to deliver care to a diverse patient population

Qualifications

• Active North Dakota Physical Therapy license (or ability to obtain before start)
• BLS certification (American Heart Association)
• Current valid driver's license and auto insurance
• At least 1 year of home health experience preferred
• New travelers welcome - no previous travel experience required!
• Comfortable working independently and managing your own schedule

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Proficiency with HomeCare/HomeBase or similar EMR systems
• Excellent communication and patient education abilities
• Ability to adapt to different home environments and patient needs
• Time management and organizational skills to handle 5-7 patients per day
