Location: Louisville, KY

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct initial start of care and recertification visits
Perform ongoing daily home health visits for patients
Assess and evaluate patient conditions and progress
Collaborate with clinical manager and educators for support
Document patient care and outcomes accurately

Qualifications

Active Physical Therapy license in the state of Kentucky
BLS certification required
Minimum 1 year of experience preferred (new grads considered)
Ability to work independently in a home health setting

Knowledge & Skills

Home health visit procedures and documentation
Management of CHF, orthopedic, and pneumonia patients
Excellent communication and interpersonal skills
Time management and self-discipline
