Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift (8:00 AM - 5:00 PM)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in Tehama County
Conduct initial evaluations and develop personalized treatment plans
Perform SOC visits for orthopedic patients when nursing is not ordered
Collaborate with the care team to optimize patient outcomes
Document patient progress in HCHB (HomeCare HomeBase) EHR system
Adapt treatment techniques to meet the unique needs of geriatric patients with multiple comorbidities

Qualifications

Active Physical Therapy license in California (or ability to obtain)
BLS certification from the American Heart Association
Home health experience preferred - we will consider travelers with less than 2 years if they have home health background
Comfortable driving up to 40 miles per day within the service area

Knowledge & Skills

Proficiency in HCHB (HomeCare HomeBase) EMR
Expertise in orthopedics, joint replacements, and geriatric care
Strong clinical reasoning and problem-solving abilities
Excellent communication and patient education skills
Ability to work independently and manage a daily visit schedule
