Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift (flexible hours based on patient visits)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments in home settings
Develop and implement individualized treatment plans
Provide skilled physical therapy services to orthopedic and geriatric patients
Educate patients and families on therapeutic exercises and safety
Collaborate with the interdisciplinary team for optimal outcomes
Document patient progress using HCHB EMR

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred; travelers with less than 2 years experience considered with home health background
Willingness to travel within Tehama County service area

Knowledge & Skills

Compassionate and patient-centered care approach
Ability to work independently in home health settings
Excellent communication and documentation skills
Familiarity with HCHB (HomeCare HomeBase) EMR
Knowledge of orthopedic and geriatric conditions
