Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover from joint replacements and manage chronic conditions like CHF and COPD
Complete approximately five meaningful visits each day while building genuine connections
Document care using HomeCare HomeBase EMR on the tablet provided by the facility

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than two years if they have Home Health background
Comfortable traveling within the service area and managing a daily mileage of approximately 40 miles

HomeCare HomeBase (HCHB) EMR system
Home Health physical therapy protocols
Orthopedic and geriatric patient care
Independent decision-making in home settings
Tablet-based documentation and mobile charting
