Location: Monticello, Iowa

Start Date: 05-10-2025

End Date: 12-27-2025

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients in their homes
Develop individualized treatment plans that help patients regain mobility and independence
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Educate patients and families on exercises and techniques for continued progress
Document patient progress and adjust care plans to achieve optimal outcomes

Qualifications

Current Physical Therapist license in Iowa
BLS certification required
Epic experience preferred but not required
Strong communication and interpersonal skills
Ability to work independently in home health settings

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Electronic medical records documentation
Patient education and family communication
Time management and independent decision-making
