Contract Type: Contract

Shift Type: Rotating shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, or 10:00 PM - 6:30 AM with every other weekend and holiday required

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents in a supportive long-term care setting
Collaborate closely with the charge nurse and care team to ensure excellent resident outcomes
Monitor and support residents with conditions such as CHF, COPD, Alzheimer's, Parkinson's, and diabetes
Administer medications and treatments with attention to detail and resident safety
Build meaningful relationships in a home-like environment that values teamwork and strong work ethics

Qualifications

Active LPN/LVN license in Minnesota
Current BLS certification required
Minimum of one year of long-term care experience preferred
Strong communication and teamwork skills

Knowledge & Skills

Familiarity with PointClickCare EMR system
Understanding of common long-term care diagnoses including CHF, COPD, Alzheimer's, Parkinson's, and diabetes
Ability to work collaboratively within a multidisciplinary team
Strong attention to detail and commitment to resident-centered care
