Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments accurately and document per policy
Monitor patients’ conditions and report changes to the care team promptly
Assist with care planning and implement delegated nursing tasks
Maintain clear, timely charting and help ensure accurate handoffs
Collaborate with nurses and interdisciplinary staff to support excellent outcomes

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (required or obtain upon hire)
Completion of an accredited practical nursing program
Comfortable with medication administration, basic wound care, and charting
Successful completion of background check and any facility onboarding requirements
Prior long-term care or clinical experience preferred but new grads considered

Education

Graduate of an accredited Practical Nursing (LPN/LVN) program
High school diploma or GED

Knowledge & Skills

Strong medication administration skills and knowledge of safe practices
Basic wound care and vital signs assessment
Clear communication skills with patients, families, and the care team
Familiarity with electronic health records and accurate documentation
Ability to prioritize tasks in a busy clinical environment
Professionalism, reliability, and a patient-centered attitude
