Roles & Responsibilities

• Provide attentive, compassionate direct patient care that supports daily living activities
• Assist with bathing, dressing, toileting and mobility while preserving dignity
• Take and record vital signs accurately and report any changes to the nursing team
• Support safe transfers and use proper body mechanics to protect patients and yourself
• Maintain a clean, safe environment and follow infection control practices
• Communicate clearly with nurses, families and interdisciplinary team members to ensure coordinated care
• Document care activities promptly and accurately in the patient record

Qualifications

• Valid CNA certification in Pennsylvania (or active registry status)
• Current CPR/BLS certification or willingness to obtain
• High school diploma or GED preferred
• Ability to pass required background checks and health screenings (TB, immunizations)
• Positive attitude, strong communication skills and a team-player mindset
• Prior experience in long-term care, skilled nursing or related setting is a plus

Education

• High school diploma or GED
• Completion of an accredited CNA training program and state certification

Knowledge & Skills

• Skilled in activities of daily living (ADLs) assistance and patient hygiene
• Proficient at taking vital signs and observing/reporting clinical changes
• Knowledge of basic infection prevention and safe patient handling
• Comfortable with electronic and/or paper charting systems
• Strong interpersonal skills and ability to build rapport with residents and families
• Ability to follow care plans and direction from licensed nursing staff
