Roles & Responsibilities

Provide compassionate direct patient care to residents, ensuring comfort and dignity
Administer medications and treatments accurately and document per facility policy
Monitor and record vital signs and report changes to the RN or clinical lead
Collaborate with the interdisciplinary care team to develop and follow care plans
Assist with admissions, discharges, and transfers to keep transitions smooth
Educate residents and families about care plans, medications, and safety
Maintain clear, timely charting and ensure regulatory compliance

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (required or able to obtain promptly)
Previous LPN experience in long-term care or skilled nursing preferred
Comfortable with electronic charting and following facility policies
Strong communication skills and a team-first attitude
Up-to-date immunizations and ability to pass background checks as required

Education

LPN diploma or certificate from an accredited program
High school diploma or GED

Knowledge & Skills

Skilled in medication administration and basic wound care
Proficient in resident assessment and monitoring
Knowledge of infection control and safety protocols
Good clinical documentation and charting practices (EMR familiarity a plus)
Strong time management and prioritization during busy shifts
Collaborative teamwork and effective communication with residents and families
