Roles & Responsibilities

Provide compassionate, hands-on patient care and support basic daily living needs
Administer medications and treatments safely and accurately
Perform regular patient assessments and promptly report changes in condition
Document care thoroughly and maintain accurate patient records
Collaborate with nurses, providers, and the care team to deliver coordinated care
Educate patients and families about care plans, medications, and follow-up
Assist with care planning and contribute to a positive patient experience

Qualifications

Current LPN license in Pennsylvania (required)
BLS/CPR certification (or willingness to obtain)
Comfortable administering medications and performing routine nursing procedures
Strong communication and teamwork skills with a patient-centered approach
Ability to work an 8-hour shift; reliable attendance and punctuality
Passion for delivering high-quality, compassionate care

Education

Completion of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

Medication administration and documentation
Basic patient assessment and vital sign monitoring
Wound care and basic clinical procedures
Electronic medical record (EMR) documentation skills
Effective patient and family communication
Time management and prioritization in a clinical setting
